Steps in Becoming an Independent Provider of
Services
for People with Developmental Disabilities:
- Attend a free seminar at the Lucas County Board of MR/DD, "So You Think
You Want to Become a Provider."

- Go to
ODMRDD’s website to print a copy of the provider application.
- Attend required training provided by the Lucas County Board of MR/DD.
- Intro to MR/DD (Rights of Persons with MR/DD)
- MUI Rule
- Specialized Support
- Contact the American Red Cross to obtain training in First Aid and CPR
at 419-329-2900.
- Send completed ODMRDD application and copies of training
certificates to the address listed on the application. Please maintain the
original for your files in case of an audit. Please know that your provider
certification may take up to eight weeks for processing by the State.
- When you have received verification of your certification from the
state, send a copy of your certification to:

- Attend Provider Orientation.

- Your name will then go on the ODMRDD Provider List and you will be contacted if
you wish to participate in Provider Fairs.
